The first step in beginning your care journey is contacting us. It's alright if you aren't sure whether in-home care is the right option for you or your loved one or if you aren't quite ready to begin services yet. Simply reaching out allows us to answer any questions you may have and get the process started. Things like managing health insurance, collaborating with your medical team, and ensuring your safety are among some things that can begin right away. Your call to us is a no-hassle, no-obligation contact with someone who cares.
Consult and Case Management
Once you've decided the times is right, we'll schedule a consultation and assessment with a home health nurse. Your nurse will assess your needs, answer your questions, and perform a safety evaluation. With you and your family, you'll work with your home health nurse case manager to create a customized care plan and the nurse will take it from there coordinating services.
Matching
Your angel will be the one meeting all of your in-home needs, so we understand that it has to be someone compatible with your preferences, personality, and lifestyle. We work hard to pair our patients with the perfect caregiver and you and your family are free to meet our in-home team to ensure we've made the right match. If, for any reason, your caregiver isn't the right one, just let us know.
Begin Care
As a collaborative team, your case manager will ensure that everyone on your case fully understands your plan of care and goals. Your nurse case manager will be present to introduce your care team to you and your family and care will begin based on your needs and preferences. Whether hourly or live-in, overnight or weekends, Angels on Call provides flexible and customized care services to meet your individual needs.
Ongoing Care and Modifications
Your care will continue as long as you need it and modifications can easily be made. Your caregiver will document activities, condition, and observations and report them to your case manager regularly. Your home health nurse will continue to conduct routine assessments and followup with your medical team to adjust care as needed.
