Therapist Where You Need Them-2

A Therapist WhereYou Need Them

Physical Therapy is an Essential Home Health Role

Physical therapists (PT) are essential to the recovery of a variety of patients and help to prevent weakness and muscle atrophy that can hinder healing. PT is an integral part of your care plan and treatments may include strength training exercises, stretching, walking, and fitting or use of mobility devices. PT may be aggressive if you are recovering from a recent orthopedic surgery to improve outcomes and reduce complications and may include the use of thera-bands, continuous passive motion (CPM) machines, exercises, and mobility devices including cane or walker. The best part is, you can get the PT you need at home, either after leaving a skilled nursing facility, or in lieu of an inpatient rehab stay.
Physical therapists can also perform safety assessments and offer suggestions for managing your chronic condition. Whether you need PT several times a day or just a few times a week, your PT team will work diligently to create a plan that helps you meet your goals and improve your overall health.

Occupational Therapy Optimizes Outcomes

Occupational therapy (OT) works in conjunction with physical therapy to help you learn to apply your strength to care for yourself. OT focuses on performing daily tasks and incorporates the use of adaptive equipment if needed, all for the purpose of helping you regain independence. Whether you suffer from a chronic condition such as MS or Myasthenia Gravis, you’re recovering from a stroke, or you are experiencing general weakness, OT can be the critical element to healing.

Speech Therapy Paramount to Thriving

Speech therapy is important for both speech disorders as well as swallowing difficulties. For those with expressive aphasia or dysarthria caused by brain injury, stroke, ALS, MS, Myasthenia Gravis, or Parkinson’s disease. Your in-home speech therapist can help with mouth and jaw exercises, speech and memory exercises, and appropriate modifications to help with your swallowing issues.
If you require food or liquid modified textured requirements, your speech therapist can help you and your family with how to make the right consistency yourself.

Skilled medical professionals!

“This agency has been a helping hand to my grandfather for the past two years. Angels on Call was there every step of the way when my grandfather took a fall. We were assisted and coached through the entire hospital discharge and transition back to the home. What seemed like a stressful move was actually a seamless transition because of the skilled medical professionals at Angels. My grandfather adores his live-in caregiver and his weekly visits from his RN Case Manager has quickly become his favorite day of the week. They truly deliver the highest level of quality care and their commitment to my family and grandfather’s needs are what sets them apart from other agencies!”

In-Home Physical Therapy

Home-care physical therapy (PT) offers the same great benefits of PT in your home rather than staying in a Skilled Nursing Facility (SNF), rehab, or traveling to physical therapy appointments at your local therapist’s office. In-home physical therapy is ideal for people who:

  • Have a condition that makes leaving the house a significant risk,
  • Experience limited mobility to the point that traveling to therapy is not safe or possible,
  • Do not have adequate or reliable transportation,
  • Choose to have private sessions at home,
  • Experience post-surgical conditions,
  • Have had a recent stroke,
  • Suffer from Parkinson’s disease, Multiple sclerosis (MS), Amyotrophic lateral sclerosis (ALS),
  • Have had a recent heart attack,
  • Are recovering from a fracture or wound.

Physical therapists and their assistants can perform all of the assessments, education, and treatments in the comfort of your home. Aside from making it convenient for the patients, there are many other benefits that include:

  • Complete care coordination including in-hospital, transition care, home care, and outpatient care if/when you choose to participate,
  • Functional applications of PT in your space,
  • Therapy on your schedule,
  • Client-centered, outcomes-driven treatment plans,
  • Coordinated care with your multidisciplinary team

In-Home Occupational Therapy

Home-health occupational therapy (OT)’s primary roles are to strengthen and restore functional abilities to allow you to care for yourself and perform activities including feeding, grooming, and hygiene. OT can help support aging seniors who have weakened over time or anyone who is recovering from an injury or illness including stroke, Parkinson’s disease, multiple sclerosis (MS), a bone fracture, or limb amputation. Benefits of in-home OT include:

  • Assessments, interventions, and treatments in the home,
  • Accurate, client-centered functional and safety assessments,
  • Personalized goals and outcomes-based treatment plans,
  • Recommendations for adaptive equipment and DME
  • Education for caregivers and family,
  • Periodic evaluation and modification of goals,
  • Cohesive interdisciplinary team approach to care.

At Angels on Call Homecare, we are proud to staff some of New York’s finest occupational therapists who deliver their services directly to your home, on your schedule. For your evaluation, connect with us today.

In-Home Speech Therapy

There are a variety of conditions and injuries that can dramatically impact your ability to speak and swallow. In-home speech therapists are equipped to help patients who have suffered from a stroke, Parkinson’s disease, multiple sclerosis (MS), myasthenia gravis, rheumatoid arthritis, TBI, dementia, and more recover their ability to speak and swallow, or modify diet texture to accommodate swallow difficulty to prevent choking and pneumonia. Some interventions your home-health ST may use include:

  • Swallow studies
  • Modified texture diet prescription
  • Breathing exercises
  • Tongue strengthening exercises
  • Sound making practice
  • Reciting common sentences
  • Naming pictures or items
  • Facial exercises

In-home speech therapy affords patients the care they deserve in the comfort of their own home. This helps promote engagement, reduce stress, and improve self-esteem and communication skills. Working in tandem with other home-health disciplines, your speech therapist can help you or your loved one reach goals.

I highly recommend working here!

“Great company to work for. I was a home health aide here for over 3 years and was always treated with respect. Pam, the owner, is a wonderful lady and treats her employees as family. The HR director, Dawn, was always so helpful whenever I had any questions or concerns. The ladies in the office are very nice and caring. I highly recommend working here if you are in need of a job as an HHA, CNA or even RN.”
